Adding Mag MAC?

hi im folowing the tutorial and cant seem to add a mag stb to the user secion of the ezserver user panel, just keeps saying failed.

I have installed the mag script and inputted the mac mac as user and serial as the password and set exp date but just keeps saying failed

please help :)
